With funding from the town of Hamden, the Public Schools and a grant from the Graustein Memorial Fund, the Partnership is focused on helping the unconnected  education, health and social services programs become a coordinated and integrated system with accountability by all.

Collaborative members identify Hamden's areas of strengths and needs, analyze the problems and identify strategies.  Understanding what parents are experiencing and what would be helpful is essential to the work of this collaborative.
